Authorization to Share Information – New Orleans Family Justice Center

Created by the New Orleans Family Justice Center and updated in July of 2009, this form allows a client to grant permission to the Center to share information with the onsite partners that s/he selects. This form can be used as a model for Developing Centers to reference while creating the Client Consent to Share Information form.
